8. It Resulted In An Explosion
An investigation later revealed that the reason why there was an explosion in the Odyssey was because of the cryo stir. The insulation on the wires which were connected to the stirrer motor in the oxygen tank was damaged because the manufacturer had not updated the design for a 65-volt operation.
In fact, the tank itself had overheated during a pre-flight test and this caused the insulation to melt completely. Naturally, on becoming heated again, there was a short circuit that resulted in the insulation igniting entirely. The fire increased pressure and caused the tank to fail.
